Internship Forms

Instructions: Students are responsible for making sure all of the required paperwork is completed and submitted by the stated deadlines for their internship. Students should keep one copy of every completed form for their own files, and are required to submit two copies to Professor Kamoie, the Internship Coordinator for the Department of History. The originals of forms requiring signatures should be submitted to Professor Kamoie.

Registration Forms for Internships or Cooperative Education. Complete both the Registration Form and the Consent and Release Form--both forms are required for registration. You can also find these forms in the back of the Schedule of Classes bulletin. Please obtain the signatures on this form in the following order: 1) your academic advisor, 2) Professor Kamoie (faculty member advising the internship), 3) Professor French (department chair). After you have obtained all signatures, turn this form into the history department office.

There are FOUR forms you need to complete in addition to the course registration and consent and release forms:

Internship Position Description, due by the first week of classes of the semester in which you are doing the internship. Alternately, you may have your supervisor at the internship write a letter that includes all of the information solicited on the form.

Student Data Sheet, due the first week of classes of the semester in which you are doing the internship.

Goal Setting and Expectations -- complete and arrange a time to discuss it with your supervisor at the internship. Signed copies due by the end of the third week of classes of the semester in which you are doing the internship.

Provide your supervisor with the form Employer Evaluation of Intern. Your supervisor should complete this form and discuss it with you before submitting it to the History Department Internship Coordinator. Both your supervisor's and your signatures are required on the form. Forms are due to the Internship Coordinator by the last day of classes of the semester in which you are doing the internship. Failure to meet this deadline may result in the assignment of "Incomplete" grades.
