This form is for individuals seeking CARD ACCESS to rooms. This is NOT for room reservations.
NOTE: In order to process your request, you must provide all 9 digits of your AU ID. This includes the 7 main AU ID #s and the additional 2 digits listed after the ID #. Typically the last 2 digits are 00 or 01.
