If you wish to request interpreter services, please request through our online form or download the Interpreter Request Form and return it to Disability Support Services. You can fax the form to 202-885-1160.

Request for Testing Accommodations

If you wish to request testing accommodations, please download the Request for Testing Accommodations and return it to Disability Support Services. This form is due one week prior to the exam date.   

Accommodation Letter Request Form

If you wish to complete an Accommodation Letter Request, please complete our online form or stop by DSS and complete a paper form.
