The following are online forms for the Disability Support Services department: Interpreter Request, Request for Testing Accommodations, and Accommodation Letter Request.
Interpreter Request Form
If you wish to request interpreter services, please request through our online form.
Requests for Testing Accommodations Form
Please use the following link to submit an online request form for testing accommodations. This form must be submitted no less than one week before an exam. Requests for final exams must be submitted by the published deadline (usually three weeks before the end of classes).
If you miss the deadline to use the online form, late forms must be picked up in person between 9:00 am and 5:00 pm from the Disability Support Services office in MGC 206 and returned to the Disability Support Services office by 5:00 pm two working days (Monday-Friday) before the exam. There is no guarantee that we will be able to secure a proctor after the week's deadline has passed.

If you wish to complete an Accommodation Letter Request, please complete our online form or stop by DSS and complete a paper form.
