Health Assessment September 2018 - August 2019

Health Assessments

Take the confidential online health assessment and get your personalized wellness report. For the most accurate score, it is helpful to have the results from your latest health screening or annual physical on hand. Your health assessment personal report identifies what you're doing well and which health behaviors may need improvement in the following areas:

  • Coronary Risk
  • Cancer Risk
  • Diabetes Risk
  • Osteoporosis Risk
  • Nutrition
  • Fitness
  • Stress and Coping
  • Safety
  • Weight Management

The report also gives recommendations on follow-up steps you should take including seeking medical advice.

Give us 15 minutes, we'll give you a reward
