HEALTH ASSESSMENT Kaiser Participants

Kaiser Permanente's kp.org is a comprehensive online resource, supporting faculty and staff in living well and achieving healthy lifestyle goals. Eligible faculty and staff can log on anywhere, anytime.

Take the Health Assessment Now

  1. Visit https://healthy.kaiserpermanente.org/health-wellness/health-assessment on a computer or mobile device.
  2. Log in using your username and password. If you are a new user, click on Create my account and follow the steps.
  3. Complete the online confidential questionnaire. You will need about 20 minutes to complete the assessment.

Kaiser's Wellness Programs are administered by Rally Health Inc., an independent company that provides health improvement management services in partnership with Kaiser Permanente for Kaiser participants.

Eligibility

Kaiser Permanente Total Health Assessment is available to all faculty and staff covered by Kaiser Permanente medical benefits.

The AhealthyU Living program is not health insurance and does not include coverage for medical services. To receive medical benefits faculty and staff must enroll upon hire or during Open Enrollment.

General Information

The confidential questionnaire is your personal health assessment, providing you with a snapshot of your overall wellness, complete with personalized results. The questionnaire asks about your daily lifestyle habits, including nutrition, physical activity, stress, tobacco use, personal safety, as well as medical history, preventive screenings, any current health conditions, and results from recent medical tests. The questions help summarize your overall wellness and determine if any lifestyle habits or factors are creating health risks. Upon completing your questionnaire, you will receive a score and customized action plan to help you succeed in creating a healthier lifestyle.

The journey to better health and wellness starts with programs found on www.kp.org. The suite of wellness services and programs are designed to empower you with the information, tools and support you and your family need to take charge or your health. The tools found on kp.org help you address a wide spectrum of needs all faculty and staff covered by Kaiser medical benefits.

The Well-Being Assessment is available from September 2020 - August 2021.

Before you visit www.kp.org, be sure to have your Kaiser member ID available 

Yes. You are able to access the questionnaire from any computer or portable electronic device and take advantage of the wellness tools and resources when it is convenient for you.

If you forgot your password, click "forgot your password/login" hyperlink and follow the prompts to retrieve/update your password.

Online Features

If you have recently had blood work done, you will want to use your lab results to answer some of the questions to get a more accurate assessment. However, lab work results aren't necessary to complete the assessment. You can log into to your electronic medical record, kp.org, to view your most recent lab results.  

After you have completed your Total Health Assessment you will receive Customized Action Plan. Take advantage of the personalized tools, support and other resources the Wellness Program gives you. The Customized Action Plan gives you simple actions to take and trackers to use for your focus areas (fitness, nutrition, stress management and more). To share this action plan with your Kaiser Permanente health care team, simply print the results and share your information with them at your next office visit.

There are many tools available to you including:

  • Review of lab and other test results
  • Secure Message your provider health questions
  • Schedule an appointment
  • Video Visits with a Board Certified emergency medicine doctor
  • Online Courses: Relax (stress reduction), Overcoming Depression, Nourish (healthy eating), Care for your health (chonic condition management), Back Pain Management, Chronic Pain Management, Diabetes Management, Breathe (tobacco cessation), and Balance (weight loss).

Wellness coaching is a service provided to Kaiser Permanente participants through convenient phone sessions. A Wellness coach gives personal guidance to help you achieve your specific wellness goals. A wellness coach is a health professional dedicated to teaching you new techniques to support positive change. They are a health education professional skilled in motivation counseling techniques. All Kaiser Permanente wellness coaches are trained to provide coaching services based on clinical guidelines and adult learning theory.

Wellness coaching is a service provided to Kaiser Permanente participants though convenient phone sessions. A wellness coach gives personal guidance to help you achieve your specific wellness goals. You can check in with your coach periodically to get the help you need to stay on track.

You can make an appointment with a wellness coach by calling 1-866-862-4295 Monday through Friday from 10 a.m. to 10 p.m. Eastern time. During your first appointment, your coach will ask you a variety of questions about your health, and help you assess your readiness and motivation to make a change. You'll choose the health topic you want to focus on, figure out if you're ready to make adjustments, and start setting goals for healthy change. Your coach can also connect you with other Kaiser Permanente resources that can help you achieve your goals, such as onsite classes and Web-based programs.

You can schedule your initial and follow up appointments when it's convenient for you. Appointments are available Monday through Friday from 10 a.m. to 10 p.m. Eastern time. 

You and your coach will determine the exact frequency and number of phone sessions based on Kaiser Permanente guidelines and your unique situation. Your coach may also put you in touch with other resources that will help you continue to make progress with your goals.

Kaiser advises that the Online Portal is a secure website, and authenticates both you and your computer before you gain access to the website.

No. Participating in the program, including the questionnaire, and health coaching, will not have a negative impact on you, your family, your job at American University, or your healthcare costs. These health and wellness resources are confidential and meant to help you live your healthiest life possible. 

No. American University does not have access to any individual answers or results. Kaiser Permanente will share the list of faculty and staff who have completed the Well-Being Assessment for distribution of rewards.

The online portal offers multiple ways to help you manage your healthcare costs. You can get a head start on great health by using the wellness tools and resources found on the online portal. First, to understand your overall wellness, complete the Total Health Assessment questionnaire. After completing your confidential questionnaire, you will receive a Well-Being Score and insights that can help you improve the different areas of well-being, including your emotional and physical well-being, healthy behaviors, work environment and more.
